Answer 1 - A Rachel Levine. Wikipedia tells us this:

"Rachel Leland Levine is an American pediatrician who has served as the United States Assistant Secretary for Health since March 26, 2021. She is also an admiral in the United States Public Health Service Commissioned Corps.
Levine is a professor of pediatrics and psychiatry at the Penn State College of Medicine, and previously served as the Pennsylvania Physician General from 2015 to 2017, and as Secretary of the Pennsylvania Department of Health from 2017 to 2021.
Levine is one of only a few openly transgender government officials in the United States... (and) became the first openly transgender four-star officer in the nation's eight uniformed services.

In the 29 March 2023 edition of The Daily Express, J.K. Rowling condemned gender transition in childhood as follows:

Question 5 - A Wikipedia tells us that Labour Party activist, Lily (Liam) Madigan, was the first openly trans 'woman' to hold the position of Constituency Labour Party Women's Officer, having been elected in November 2017 in Rochester and Strood.

A complaint by Madigan alleging transphobia against the previous officeholder, Anne Ruzylo, was not upheld but had led Ruzylo to standing down.

In 2016, Madigan took legal action whilst still at school after it made her wear a male uniform, continued to call her Liam and denied her access to female toilets.

Madigan ran the LGBTQ+ liberation group at the University for Creative Arts, the women's liberation group, and was also Campus Executive Officer.
